Summons on Return – This is the document type to select if the document your are filing is a SUMMONS (on Return) with the attached Affidavit of Service. In other words, you have a single PDF document which includes both the Summons and the Affidavit of Service.
Summons – To Issue – If you would like the Court to ISSUE a summons, which is not typically necessary because Utah attorneys are considered officers of the court and are able to issue their own summonses. In the future, when Pro Se filers are able to e-file they will have a need for this document type.
