When filing a new divorce or adoption case, you’ll need to submit the UDOH Certificate/Vital Statistics form. Pay special attention to the Additional Info section where you’re asked to enter the number of persons. Currently, the courts require a numerical value in this box, even if there are no persons/children involved in the case. If this applies to your submission, simply enter “1” in the box to designate no persons/children are involved. This will allow your filing to be processed successfully.
