The Court now requires that a vital statistics form be filed on new case initiation for Divorce and Adoption case types. The document will need to be saved in PDF format.
When filing a new divorce/adoption case, you file the UDOH Certificate/Vital Statistics form. The filing under section 7 says to enter the number of children. At this current time you need to fill the box in with a number, even if there are no children. If no children, put the #1 in the box. This will enable you filing to go through.
